Phi Lambda Sigma is the international pharmacy leadership society. Our mission is to support pharmacy leadership commitment by recognizing leaders and fostering leadership development.

Chapter Reporting Forms

The Chapter Report is used by the PLS Board of Directors to assess trends among chapters, to develop programming to strengthen our collegiate members, and to determine the Chapter of the Year Award. 
All Chapters should submit an annual Chapter Report to the National Office no later than August 31st at 11:59 pm PT.  
If your chapter conducts events outside of a traditional academic year (ie July/August-May/June), you may choose when to report activities but they should not be included on multiple years’ reports.

You will submit your report on the google form linked above.  Click HERE for a “View Only” version of the Chapter Report Form to view all sections of the form at once in preparation for your final submission.

This form is to ensure the continuity of contact between the Phi Lambda Sigma Board of Directors and our local chapters.
While we understand that you may not have the same officer position titles at every chapter, we need the contact information and title for each outgoing and incoming officer.
This form should be filled out by each chapter, annually by August 31st in order to ensure that communication may continue between our chapters and the Phi Lambda Sigma Board of Directors.
