Kathlene A. Hawkins Memorial Travel Award
PURPOSE OF AWARD
Phi Lambda Sigma is pleased to offer travel awards to promote leadership development among our student members. The Kathlene A. Hawkins Memorial Travel Award is given in memory of Kathlene, an avid traveler, who served as our administrative support person for 15 years. Known for her timeliness and kind heart, she died suddenly in 2018 while on her annual pilgrimage to Italy. The award monies will be used to offset registration and travel costs associated with a leadership development conference/meeting. The leadership conference is not required to have a pharmacy focus.
NUMBER OF AWARDS & STIPEND
Awards up $500 are awarded on a rolling basis.
ELIGIBILITY
Applicant must be a current student that is in good standing at their School/College of Pharmacy and a member of Phi Lambda Sigma.
APPLICATION DEADLINE & SELECTION PROCEDURE
Applications are accepted on a rolling basis. The Phi Lambda Sigma Awards Committee makes the selection based upon careful review of completed materials. Applications need to be submitted at least 90 days prior to anticipated travel dates. This allows the Awards Committee sufficient time to review your application. Award decisions will be made within 60 days of each application due date.
2024 – 2025 Deadlines: September 1st, December 1st, March 1st, May 1st
APPLICATION & SUBMISSION PROCEDURES
Completion of BOTH of the following forms is required for your application to be considered complete:
- Award Application Form
- Chapter Advisor Attestation Form
The Award Application Form should be completed by the applicant, and the Chapter Advisor Attestation Form must be completed by the applicant’s PLS Chapter Advisor. Both forms are linked below.
RECEIPT OF AWARD MONEY & REQUIRED REPORT
Recipients of the award money are expected to submit a report no later than 30 days after the meeting. The report should include 2-3 photos from the event (with captions) and a 250-word description of the event. The report should include an overview of the conference and attendees, specific leadership development concepts covered during the conference, and how this leadership development will benefit the awardee’s chapter, school, the profession of pharmacy, and him/her as a professional. The report should be submitted to awards@philambdasigma.org.
