Alumni of the Year Award

To recognize alumni of Phi Lambda Sigma who have exemplified leadership qualities in support of the profession of pharmacy.

Number of Awards and Recognition

Chapters or PLS member individuals may submit nominees for the PLS Alumni of the Year Award. The recipient of this distinguished recognition will receive an engraved plaque and an award of $500 to an organization of the recipient’s choice. The award will be presented during the Phi Lambda Sigma awards ceremony and the recipient will be given the opportunity to make a brief address.

Eligibilty

The individual chosen must have a proven record of leadership accomplishment in any one or more of the following areas: professional practice, education, association work, community service, or mentoring. The individual must be a member of Phi Lambda Sigma. The nominator is responsible for completing the nomination form and submitting the award nomination packet. Self nominations are accepted. The nomination packet is to include the following items:

• Nomination form
• Current Curriculum Vitae of the nominee
• Up to 3 letters of recommendation. At least 1 letter of recommendation must come from a PLS member.

Please complete the Alumni Member of the Year Nomination. The deadline for submission is December 15th at 11:59 pm PST annually.
